You see servers crash often from power issues. Power surges hit the system hard. Supplies burn out without much notice. Heat rises fast in the racks. Fans quit working after some time. Dust piles on and blocks airflow. I have seen this wreck things quick. You need to check connections regularly. Or maybe test voltages with tools. But power problems sneak up easy.
Also software bugs cause lots of trouble. Updates go wrong during installs. Drivers clash with new patches. Memory leaks eat up resources slow. Processes hang and freeze the whole thing.
